SUPPLIER EVALUATION QUESTIONNAIRE

Suppliers of goods and/or services that are deemed to pose a risk to Karratha Contracting’s (KCPL) employees or business, must complete this questionnaire and provide documents nominated via email to admin@kcpl.net.au. Unsuccessful applicants will not be engaged by KCPL.

The applicant is expected to abide by KCPL’s Supplier Code of Conduct, which sets our minimum HSEQ and labour requirements, including those set by our clients when on site. The supplier is responsible for ensuring all relevant Company Insurances and Licences are current whilst engaged with Karratha Contracting. If Suppliers engage/subcontract any works, they accept complete liability for works completed and/or that Contractors will hold the relevant Insurances and Licences for works undertaken.

Records relating to supply and compliance must be retained for a minimum of five years after works have been performed, or as legally required. Such records must be made available to KCPL on request.

Works must not commence until a valid KCPL Purchase Order is provided. Claims are to be submitted in a valid GST tax invoice format, clearly stating the amount payable, and details of the works completed. All invoices must be emailed through to admin@kcpl.net.au for processing, failure to do so may delay payment. Terms of payment are strictly 30 days End of Month.

Section 3: Licences, Registrations & Personnel

Please provide details of all Statutory Registrations and/or Licences held by your organisation and provide copies of each with the submission of this questionnaire. (i.e. Electrical, Building, Plumbing, Pest Control, Gas Fitting).
All personnel attending KCPL job sites, must complete the KCPL induction and provide the following mandatory information prior to commencing works:

  • Photo ID, such as Driver’s License
  • White Card
  • Trade Licence/s (if applicable)
  • Any other licenses/qualifications, such as High-Risk Work License, First Aid, MSIC, other induction cards/records etc
  • Emergency Contact Details

All workers must adhere to KCPL PPE requirements and the PPE requirements of our clients. Minimum PPE is long sleeved shirts, long trousers and lace up safety boots. Depending on the site requirements you may need to provide a clear drug & alcohol screening. Please note your staff may be required to undertake random D&A screenings and we operate with a strict zero tolerance for drugs and alcohol in the workplace.

The KCPL Drug & Alcohol Policy forms part of the KCPL Code of Conduct. Sub-contracting personnel who fail to comply with this policy will be in breach and we reserve the right to terminate all contract works immediately.

Please list all personnel expected to work on KCPL sites and provide copies of their qualifications/licences with your submission. It is a mandatory requirement that all personnel complete the KCPL induction PRIOR to working on any site/s.
